You Are The Pilot
There is no one that knows you; more than how you know yourself. 
When you choose Support & Care Alliance to work with you, we follow your support plan to accomplish the goals that you have set in your NDIS Plan and follow your lead as well in every aspect of your plan. Rather than delivering support to you, we will deliver all our support to you and we will become a part of your support network system. We will assist you to recognise and negotiating your support to enable you to make the right choices based on consistent and up-to-date information always at your disposal.  

At Support & Care Alliance, we are committed to upholding your human rights, respecting and inspiring your rights to make decisions. 
With your involvement, we will customise support with you, listen to you, check we have understood your NDIS plan, plan with you and support you to reach your goal.

Our Person-led supports
At Support & Care Alliance, we promote a relationship base model that put you at the centre of how you want to be supported. Our responsibility is to professionally provide the service or services you have requested with the utmost care, dedication and accountability. You will always be the centre of your life’s decisions. Your partnership with us will open up a world of possibilities; that will be beneficial to you and your family members. Our team members will support you to set your goals, learn to make the right decision that favours your interests and solving problems to live the life you want to the fullest.   

These decisions include; how, when and where your support is delivered, what and who is delivering your support, and what is included in your support system.
Your support is designed with you and will be about you. While we co-design the supports and services you need, you and/or your family members will be fully involved in the process. These tasks may involve asking you a set of targeted questions in order to fully understand your needs and support.
 
Examples of the questions will be:
  • ­What do you want to do, learn, and achieve?­
  • What do we need to do to support you to do, learn, and achieve your goals?­
  • Who are the people with the right expertise to engage to support you?
  • What resources do you want and need?
  • What can we change and plan for, so you can reach your full potential?
At Support & Care Alliance, we have specialised knowledge about the NDIS; based on our experience. We can design plans that support your goals and support needs. Tell us what you want, and we will model plans that include all your support needs. Working with you and people close to you, such as family members, and staff. We will match your support with your unique circumstances, preferences, culture, gender and personality.
We will match you with the right workers. Your team of support staff members are very important as much as the support they provide to you.

Before we assign support workers to you, we will have a formal discussion with you and ask a series of questions about what qualities and skills you want in your support staff. 
Based on the information we receive from you, maybe you probably want a staff member from a cultural background, ethnic group or even a staff member with the same interest and values you have. Also, you will probably need a support staff with a car that can accommodate your wheelchair while he drives you to the shops or in the community and takes you on your outings in the community. At Support & Care Alliance, our person-led approach will make you the centre of designing your support plans. This means that you can select staff members you want to work with you, based on your needs and their experience.

At Support & Care Alliance, we see the individual; not their disability. We focus on the ability you have, no matter how small it is. We will also work with you on the areas you may like to see improvement in and support you to learn new skills.

Tailored support involves the participants in the whole process by listening, checking and making sure they understand the process, planning and enabling and evaluating with participants to ensure their complete understanding.

Every support & service we provide is modelled and delivered how you want it, whenever you want it & wherever you want it. You are the director of your life, our role is to focus on the life you want, so you can live it & improve your independence.

Support & Care Alliance provides services to participants as well as participants’ loved ones, notably the people you care about, people you want to see progress in life and people you want to see live a more independent life. So, get you and your family involve, we are here to assist.  

Culturally & Linguistic Diversity Strategy

We recognise that people with disability from Culturally and Linguistic Diversity backgrounds can face additional challenges in terms of inclusion in their communities & this limits their ability to access the support they need. 

 

Advocacy
People with disabilities must be accepted and respected in their workplaces, social settings, community centres & educational facility. We will continue to advocate across all levels of government & non-government agencies so that you enjoy the same rights and opportunities as everyone else.
