Respite Services

Support & Care Alliance provides short-term accommodation in line with a person-centred approach set by your plan. This service is designed to suit individual needs, care and choices. Our trained staff provide support in a number of support items, depending on your needs and goals.
This includes:

Magnolia Centred Care has motivated support workers to assist you in achieving your personal and health goals. From helping you around the home by assisting you with household tasks like washing dishes, cooking, laundry, mopping, cleaning toilets and bathrooms. Magnolia Centred Care staff will assist you in attending appointments and helping you to maximise your independence.
 

Household Tasks

Assistance with Travel & Transport

 
Assist / Travel Transport is specifically designed for participants who cannot use public transport or are unable to drive their own car; due to their disability. Support & Care Alliance offer travel arrangements and support for people to access essential services in their community and beyond. Our services include travel arrangements, in corroboration with the public transport system or participants’ private car to attend school daily, educational/learning facilities, employment, appointments, family visit, leisure events, holidays (only in Victoria) or community activities. We have highly qualified transport professionals with valid and full driver’s licences, they have been cleared fit to work by NDIS Quality and Safeguards Commission and also have authorised police background checks.
Our services include,
  • Transportation to the shops and assisting with grocery shopping
  • Transportation to and from day services
  • Transportation to recreational centres
  • We can transport you for family visitation

 

Community Nursing 

The objective of Community Nursing Care is to provide specialist services for participants with high care needs that required a high level of skill by registered nurses. Our extremely skilled registered nurses promote an exceedingly healthy lifestyle and provide nursing disability care services both when participants fall ill and provide mechanisms to prevent illness. Support & Care Alliance Community Nurses are equipped and trained to provide diverse nursing procedures such as:
  • Wound care (including cleaning and dressing)
  • Checking temperature, blood pressure and pulse
  • Specialist administration of injections
  • Supporting doctors with medical examinations and medical procedures

Daily Personal Activities

Support & Care Alliance emphasises your mental well-being in as much as your physical state. Daily Personal Activities High Intensity

 

High-Intensity Daily Personal Activities is described by NDIS as the more complex support some individuals might need to manage their daily lives on a daily basis. Our registered nurses and trained support workers are qualified in supporting our participants with complex healthcare needs. Support & Care Alliance is committed to providing a high standard of care and delivering high-risk support to participants who require high-intensity daily personal activities to meet their healthcare needs.
We can come to your homes or in community health settings and support you with the following.
  • Complex bowel care
  • Tracheostomy management
  • Sub-cutaneous and medication management
  • PEG feeding and Management
  • Ventilator management
  • Urinary catheter Care
  • Diabetes management

Participants are expected to have a healthcare plan specific to their needs, care and support, we will apply a person-led support approach. Our participants will be fully involved in the planning and managing of their care and support needs.

Group & Centre-Based Activities

Support & Care Alliance assist participants to participate in the ground-based community, social and recreational activities of their choice. Our services include accompanying participants to movies, beaches, art and crafts, outdoor activities with their peers, and to explore Melbourne City and its surroundings. Enjoy a variety of social experiences, activities and develop your skills.

Development of Daily Living Skills

Our Development-Life Skills program is designed to encourage participants to develop individual life skills to help them gain independence, personal knowledge and confidence. Basically, this includes special personality development life skills, behavioural life skills development, and daily living support services. We provide a variety of training to participants such as public transport training and support, developing skills in their community and with peers, recreational and social participation.  Support & Care Alliance will engage with participants, recognise goals and work with you and your family or carers to initiate support and develop your everyday skills with the aim of building independence and confidence.


Assistance with Life Stage Transition

 

Adjusting to a change of environment can be the easiest when you have someone to help you with your transition. Support & Care Alliance has kind-hearted support team members to assist participants with the transformation of implementing the plan by establishing the ability to connect with numerous other support systems. We also assist participants to have clear knowledge and understanding of the purpose of funded support and their inclusion in the community.
Some of the aspects Support & Care Alliance can assist participants in are:
  • Goal and life planning
  • Peer support, individual skill development and life transition planning including mentoring
  • Assistance with decision-making in a non-coercive fashion
  • Assistance with establishing networks in a participant’s circle
  • Development of independent self-care and living skills
  • Daily transportation

Assist Access / Maintain Employment

Support & Care Alliance will design an employment support plan and create customised training plans; specifically tailored to the participants’ goals to maximise participants’ achievement of their desired results. We will create a person-centred individualised support plan to assist you or your loved ones to transition from school to work smoothly; while engaging with families and carers around the participant’s goals.
Depending on the participant’s employment aims and goals/and their current skill level. We can assist to develop and/or assist in ‘soft and hard skills’ which may include, but are not limited to:
  • Resume preparation and job engines training
  • Assist to prepare you for interview
  • Accountability
  • Social and business communication
  • Building resilience
  • Workplace norms, behaviours, ethics and expectations.
  • Travel arrangement and support to travel to and from the workplace
  • Recruitment paperwork preparation
  • Preparing for a participant’s first day, including induction at the workplace
  • Planning and organising basic necessities to be ready for work

Participating in community-based activities fosters good health and well-being as well as connection to other people in your community. The staff at Support & Care Alliance instils a strong sense of community participation by supporting participants in activities; that will assist them in building confidence, making friends and learning new skills.
Participation in the Community
This program involves social and community participation for people living with disabilities. In line with your NDIS funding, our staff at Support & Care Alliance have the resources to facilitate your involvement in the below activities independently.

In-Home Support/Personal Care


 

Our trained staff are available to work with you in a Specialised Disability Accommodation (SDA), community centres or in your private home. This includes providing support to assist you around your home.
Support & Care Alliance staff will assist you with the following:

  • Assisting you to get ready for the day
  • Preparing meals for you and enjoying your life without having to depend on others
  • Personal hygiene
  • Shower, grooming, laundry and other miscellaneous support you need

Assistive Equipment for Recreation / Vision Equipment


 

Support & Care Alliance provides a variety of recreation-related supports, needs and care, from assisting participants to change into sports gear to assisting with travel arrangements. We focus on specialised recreational equipment and modifications to recreational equipment to enable participants’ enjoyment of activities. This can range from modified sports shoes to assistive music technology. An example of assistive music technology is a product known as Skoog which is used to provide music therapy to people/or children with developmental disabilities, by allowing them to play music with their hands, elbows, head and knees.  
 

Assistive Products for Personal Care and Safety / Personal Mobility Equipment



NDIS designed this service to assist participants with low-cost products and services. At Support & Care Alliance, we understand that it can be a daunting task to look for certain products in the ever-enlarging markets. Our mobility-trained staff members will assist you in finding the right products to suit your needs. Using your Assistive Technology funding, we help participants to source readily identified and locally made products that will be integrated into their life for effective low-risk use.
This includes but is not limited to products such as:
  • Small devices to assist with dressing and undressing
  • Eating and drinking aid
  • Replacement mobility stick/cane
  • Replacement covers
  • Elbow crutches
  • Walking sticks and canes, shower stool or bench
  • The adjustable toilet seat raiser
  • One-off/short-term supply of washable briefs
  • Pad, bedding, kylies
  • Basic bedside commode, bath transfer bench, grab rails
 

Communication and Information Equipment



 
Support & Care Alliance will assist to find products and technologies that provide alternative communicative methods for our participants. These include products that help participants access spoken or written communication. By making use of these products, participants can build capacity, improve confidence, and increase independence. Our trained staff will assist you to achieve these goals.
